In consideration of being allowed to participate in training, practices, games, camps, or any other activities (“Activities”) provided by KT13 LLC, the undersigned agrees as follows:
1. Assumption of Risk
I understand that participation in baseball involves inherent risks, including but not limited to physical injury, illness, collisions, equipment failure, weather conditions, and other unforeseen hazards. I voluntarily assume all risks, both known and unknown, associated with participation in these Activities.
2. Release of Liability
I, on behalf of myself, my child, and our heirs, executors, and assigns, hereby release, discharge, and hold harmless KT13 LLC, its owners, coaches, staff, volunteers, and affiliates from any and all liability, claims, demands, or causes of action arising out of or related to any injury, illness, damage, or loss sustained during or in connection with participation in the Activities, whether caused by negligence or otherwise.
3. Medical Treatment
I authorize KT13 LLC staff to provide or arrange medical treatment in the event of an injury or illness. I agree to be financially responsible for any medical expenses incurred.
4. Insurance Responsibility
I understand that KT13 LLC does not provide medical or accident insurance for participants. It is my responsibility to provide such coverage for my child.
5. Photo/Video Release
I grant permission for my child’s image, likeness, and/or voice to be used in promotional materials, social media, or other media produced by KT13 LLC without compensation.
6. Acknowledgment of Understanding
By signing below, I acknowledge that I have read this waiver, fully understand its terms, and voluntarily agree to its provisions. I understand that I am giving up substantial legal rights, including the right to sue.
